At its October meeting, the ACGME Board of Directors approved the following:
New Program Requirements for Graduate Medical Education in Interventional Pulmonology, effective October 18, 2023. The Requirements are available under the subspecialty name under the Program Requirements and FAQs and Applications tab of the Internal Medicine section of the ACGME website. The new program application is available on the same page.
Interim revision to Program Requirements for Graduate Medical Education in Anesthesiology Critical Care Medicine, effective July 1, 2024. The Requirements are available under the subspecialty name under the Program Requirements and FAQs and Applications tab of the Anesthesiology section of the ACGME website.
Major revision to Program Requirements for Graduate Medical Education in Adult Congenital Heart Disease; Advanced Heart Failure and Transplant Cardiology; Cardiovascular Disease; Clinical Cardiac Electrophysiology; Critical Care Medicine; Endocrinology, Diabetes, and Metabolism; Gastroenterology; Hematology; Hematology and Medical Oncology; Infectious Disease; Interventional Cardiology; Medical Oncology; Nephrology; Pulmonary Disease; Pulmonary Disease and Critical Care Medicine; Rheumatology;and Transplant Hepatology, effective July 1, 2024. Interim revision to Program Requirements for Graduate Medical Education in Nuclear Medicine, effective July 1, 2024. The Requirements are available under the specialty name under the Program Requirements and FAQs and Applications tab of the Nuclear Medicine section of the ACGME website.
Interim revision to Program Requirements for Graduate Medical Education in Pain Medicine, effective July 1, 2024. The Requirements are available under the subspecialty name under the Program Requirements and FAQs and Applications tab of the Anesthesiology, Neurology, and Physical Medicine and Rehabilitation sections of the ACGME website.
To align with these revisions, the Selected Topics Across Program Requirements documents for Coordinator Dedicated Time, Core Faculty Dedicated Time, Program Leadership Dedicated Time, Number of Faculty, Program Director Qualifications, Resident/Fellow Scholarly Activity, Direct Supervision Using Telecommunication, and Independent Practice have been updated and can be found on the Institutional Applications and Requirements page under the "Selected Topics Across Program Requirements" heading.
Note: The proposal to make an interim revision to the Adolescent Medicine program requirements pertaining to the length of educational program for internal medicine graduates has been withdrawn, pending further consideration.

Join the 2024-2025 ACGME Equity MattersTM Learning Community
The ACGME seeks organizations to participate in the 2024-2025 ACGME Equity Matters Learning Community. Organizations engaged in this new Learning Community should be committed to developing robust, sustainable approaches focused on increasing physician diversity and providing inclusive learning and working environments.
Applicants will form an organizational team and commit to engaging in the 18-month program and producing a capstone project.

NEW! 10-Year Accreditation Site Visit and Self-Study Updates for Programs
Effective October 2023, the ACGME has officially discontinued 10-Year Accreditation Site Visits for programs. While the Self-Study is still required (Common Program Requirement V.C.2.), it will no longer be linked to or reviewed during a site visit. All program 10-Year Accreditation Site Visit and Self-Study dates will be removed in the Accreditation Data System (ADS) by late fall 2023.
Previously, all programs with a status of Continued Accreditation, Continued Accreditation with Warning, and Continued Accreditation without Outcomes were assigned both a Self-Study and a 10-Year Accreditation Site Visit date, and the review of the program Self-Study was incorporated into the 10-Year Accreditation Site Visit process. Between March 2020-October 2023, program Self-Studies and 10-Year Accreditation Site Visits were postponed and delinked.
The ACGME Manual of Policies and Procedures (available on the ACGME website) reflects these changes. The Site Visit and Self-Study sections of the ACGME website will be updated in the coming weeks to reflect these changes.
The ACGME will communicate once these changes are complete within ADS and on the website.

2023 Milestones National Report
The 2023 Milestones National Report is now available on the Research and Reports page of the Milestones section of the ACGME website.
The eighth edition of the Milestones National Report presents national Milestones data in aggregate form for the latest academic year (July 1, 2022-June 30, 2023). The ACGME produces this report for every academic year so program leaders can compare national-level data in their specialties and subspecialties directly to their own program data.
New for the 2023 report:
- Information on Osteopathic Recognition Milestones.
- For easier use and distribution, specialty-specific publications, including the applicable subspecialties, will be posted on each specialty's Milestones tab on the ACGME website by mid-November.
Visit the Milestones section of the ACGME website for additional online resources and tools including the Milestones Guidebook, Milestones FAQs, Milestones Guidebook for Residents and Fellows, and the Clinical Competency Committee Guidebook.
NEW! Case Log Update for Ophthalmology Residents
The ACGME Case Log System has been updated to reflect advances in the treatment of glaucoma and improve the ease of logging. The glaucoma minimum category was expanded to two categories, "Minimally Invasive Glaucoma Surgery (MIGS)" and "Tube Shunts and Trabeculectomy," each with a required minimum of five. The "Tube Shunts and Trabeculectomy" minimum includes both the Surgeon and Assistant roles.
The glaucoma minimums will be in effect for the 2024 graduates but will not be enforced until 2025 to give programs time to acclimate to the change. Note: It will take at least a week for the Ophthalmology Minimum Report in ADS to be fully updated.
Residents can now “batch enter” most cataract, YAG capsulotomy, laser trabeculoplasty, panretinal laser photocoagulation, and intravitreal injection CPT codes. Additionally, Case ID entry is now optional.
Review the document, "Ophthalmology Case Log Information," which is on the Documents and Resources tab of the Ophthalmology section of the ACGME website. The document includes helpful information about the new glaucoma minimums and batch logging CPT codes.

THIS WEEK! Review Committee Chair to Present at #AAO2023
Dr. Laura Green, chair of the Review Committee for Ophthalmology, will present the session, "Shaping GME: The Future of Ophthalmology," at the American Academy of Ophthalmology (AAO) Annual Meeting Saturday, November 4, 2023, from 3:45-5:00 p.m. Pacific about the upcoming major revision of the Program Requirements for Graduate Medical Education in Ophthalmology. Dr. Green will provide updates and facilitate an interactive discussion about the ophthalmologist of the future.

Major Revision of Program Requirements for Anesthesiology Subspecialties Coming Soon
The Review Committee for Anesthesiology is preparing to begin a major revision of the Program Requirements for the subspecialties of:
- Adult cardiothoracic anesthesiology
- Anesthesiology critical care medicine
- Obstetric anesthesiology*
- Pediatric anesthesiology
*The interim revisions for the Program Requirements for Graduate Medical Education in Obstetric Anesthesiology posted for public comment on May 12, 2023 have been tabled, and will be considered as part of the major revision process now underway. There is potential for additional changes to the previous proposed revisions.
The Review Committee invites comments related to specific requirements or topics that should be addressed as the revisions are developed. In particular, the Review Committee invites comments related to what the respective subspecialties will look like in the future, and the competencies, resources, and personnel necessary to create physicians who will practice in the subspecialty. Comments should be submitted to an@acgme.org by Thursday, December 7, 2023.

The Review Committee for Family Medicine is preparing for a major revision of the Program Requirements for the multidisciplinary subspeciality of hospice and palliative medicine (view current Requirements here). The community is invited to make comments related to specific requirements or topics that should be addressed as the revisions are developed. In particular, the Review Committee invites comments about what the subspecialty of hospice and palliative medicine will look like in the future, and the competencies, resources, and personnel necessary to create physicians who will practice in the subspecialty. Submit comments to hpmrevisions@acgme.org by the extended deadline of November 23, 2023.

Medical schools lack feedback on how their graduates perform during residency, information that is helpful for continuous quality improvement of curricula. The Association of American Medical Colleges tested the feasibility of administering a national standardized Resident Readiness Survey to residency program directors about the readiness of their PGY-1 residents.
Are We Teaching Evidence-Based and Inclusive Practices in Gender-Affirming Care? Perspectives From Plastic Surgery In-Service Examinations

This study evaluated the quality of content pertaining to gender-affirming surgery included in plastic surgery in-service examinations, and found significant gaps in the educational content. There is a need for increased quality of gender-affirming surgery-related questions on plastic surgery in-service exams addressing: appropriate language use; non-traditional surgical options; adherence to established consensus guidelines; and representation of gender diverse identities.
The ACGME Seeks Public Members
The following Review Committees seek their next public members for a six-year term beginning July 1, 2024. Public members share the perspective of the public, foster public accountability, enrich the work of the Review Committee through outside expertise, and create transparency. While nominees cannot be physicians, other health care professionals are permitted if they are not employed by an organization with ACGME-accredited residency or fellowship programs.

The ACGME Seeks Osteopathic Recognition Committee Member
The Osteopathic Recognition Committee is seeking one new physician member. Candidates must be a program director or Director of Osteopathic Education in a program with Osteopathic Recognition. Nominations are due December 1, 2023.
The Recognition Committee will forward its selection to the ACGME Board of Directors for approval. The new member’s term will begin July 1, 2024 and end June 30, 2030.
